Take your nursing to the next level and provide trauma-informed care to clients presenting with a wide variety and complexity of acute and chronic illnesses/diseases, including communicable diseases, substance dependency, and mental health and general health issues.
Work collaboratively with an interdisciplinary, collaborative health care team, liaising with other health care professionals, clients, family members, and community agencies to provide clients with safe and effective care.

The community transition teams help people with mental health and opioid-use disorders transition back into their communities after release from a corrections facility. They work with clients for up to 90 days following their release to connect with a community physician, fill prescriptions, and access other community supports. We have various roles including social workers, psychiatrists, physicians, nurses, Indigenous patient navigators and more.
