Rehabilitation & Recovery Program
People who have completed treatment at θəqiʔ ɫəwʔənəq leləm’ (Red Fish Healing Centre for Mental Health and Addiction) or the Heartwood Centre for Women can take part in the program. It is located in Coquitlam, BC, and has 40 beds.
The program helps people transition back into community-based care with their local health authority. Some people find this type of support helpful.
Who is the Rehabilitation and Recovery program for?
If you are medically stable and you have completed treatment at the Red Fish Healing Centre or the Heartwood Centre, you can choose to take part in the Rehabilitation and Recovery program. It is a voluntary program.
What to expect from the Rehabilitation and Recovery program
You can expect a program tailored to your individual rehabilitation and recovery needs. Options include the following:
- Ongoing mental health and substance use assessment and support
- Crisis intervention (if you need it)
- Coordinated psychiatric and medical treatment
- Life and social skills building
- Drug and alcohol recovery and counselling programs
- Job skills training and education
- Peer support
- Advocacy
- Help with other resources you may need, for example, housing and social services
Your health care team
A team of health care professionals will work with you on your recovery. Your team might include:
- Psychiatrists
- Physicians
- Nurses
- Social workers
- Vocational counsellors
- Rehabilitation workers
- Leisure workers
- Indigenous care coordinators
Referrals
The program is by referral only. Referrals are made by Red Fish Healing Centre and Heartwood Centre for Women, in partnership with the person and their regional health authority care team.
If you are a client or patient at one of these facilities and you are interested in the program, talk to your case manager or a member of your care team.
Resources for Indigenous patients and families
If you are Indigenous, a dedicated Indigenous care coordinator (ICC) can support you and your family while you are receiving services from the Rehabilitation and Recovery Program. The coordinator ensures you have access to high-quality care that is free of discrimination. They also help empower you to determine your own health care needs.
The coordinator can support you by incorporating Indigenous wellness practices as part of your treatment plan. They can also facilitate access to Indigenous-specific programming that is culturally safe and relevant.
You can request support from the coordinator at any point during your admission. You can ask any member of your health care team to submit a referral.
The Indigenous care coordinator provides:
- Holistic care planning and Indigenous wellness programming
- Individual cultural and spiritual supports, including coordination and access to Sacred Spaces
- Client and family advocacy
- Help in addressing and/or reporting care that is harmful or discriminatory
- Health system navigation, including support with communicating to your health care team
The coordinator can also assist with:
- Discharge planning and resource coordination
- Connecting with First Nations Health Benefits or Non-Insured Health Benefits program for First Nations and Inuit
- Connecting with other sources of support available to all Indigenous patients
