Community Transition Teams
Community transition teams aim to:
- Reduce overdose deaths
- Enhance health outcomes
- Reduce harms associated with repeat involvement with the criminal justice system
There are 10 teams located across the province. The Provincial Community Transition Team Hub (the Hub) is located in the Lower Mainland. It provides coordination and standardization across the regional teams. The Hub also supports quality improvement activities as well as education and training for CTT staff.
Who are community transition teams for?
The community transition teams provide short-term bridging services to clients with complex health care needs, including substance use and mental health disorders, as they transition from provincial correctional centres.
The teams serve as health navigators. They link people to mental health and substance use services in the community. They also help bridge the gap between people as they leave custody, and the communities in which they live.
The teams provide support to people in the weeks and months leading up to their release from a correctional centre. They also provide support for up to 90 days after their release.
What to expect from community transition teams
The community transition teams use a client-centred, holistic, trauma-informed, culturally informed/sensitive and strengths-based approach to care for people.
Each community transition team includes:
- Social workers
- Peer support workers
- Indigenous patient navigators
- Registered nurses/nurse prescribers
Clients can expect comprehensive care from the team, including:
- Routine health care
- Short-term counselling
- Opioid agonist therapy, support and relapse management
- Connections and referrals to health and community services
Community Transition Teams (CTT) Provincial Program
Provincial CTT Hub
The Hub has clinical, managerial, coordination, and administrative support staff to help CTTs better serve clients with complex needs and generate important knowledge regarding best practices.
The Hub provides additional clinical support including psychiatry, psychology and addiction medicine.
