Provincial Assessment Centre Referrals
Admissions
The centre supports people who are 14 years old or older. It provides interdisciplinary, intensive assessments to support ongoing care planning.
Potential clients must be referred to the program. Admissions can be voluntary or involuntary under the Mental Health Act. All admissions are planned in advance.

Eligibility criteria
The potential client must be 14 years of age or older and:
- Have a dual diagnosis of a developmental disability, psychiatric and/or behaviour issue
- Be assigned to a Community Living BC (CLBC) facilitator or analyst, or in the case of youth, a Ministry of Child and Family Services staff member
- Be followed by CLBC, or be CLBC-eligible in the case of youth
In addition:
- Caregivers and support networks agree with the referral
- A personal support plan is in place
- They are medically stable at the time of admission
- There is a plan for the individual to successfully return to their home and community
- Everyone involved in the potential client’s care understands that PAC offers short term assessments (two weeks to three months). The program does not offer long term assessment
Community Living BC facilitators or analysts may refer clients to the program. For youth, referrals can come from Ministry of Children and Family Development staff.

Please contact the Provincial Assessment Centre at 604-660-0228 ext. 336525 or PACReferrals@phsa.ca before you fill out the referral package.

Complete the referral package
If a client is eligible, the PAC intake coordinator will send you a PAC referral package. Complete the package and forward as indicated.
The referral package includes the following sections:
- Referral application – Community Living BC analyst/facilitator or Ministry of Children and Family Development worker will fill in
- Psychiatric information – psychiatrist will complete
- Caregiver information – primary caregiver will fill in
- Consent forms – potential client or primary caregiver will sign
- Supporting collateral documentation and historical records as requested/required

Review
The PAC's clinical team will review the referral package and decide if the potential client is a suitable admission or not. The PAC intake coordinator shares this decision with all people involved in the referral.

Pre-admission care planning meeting
If the client is accepted, the PAC intake coordinator schedules a pre-admission care planning meeting between PAC staff and the client's support team.

Prepare for admission
The coordinator shares the assigned admission date to all people involved in the referral. Before the admission date, clients or their caregivers will be given information to help them prepare for the admission.
